Step 2:
Forms will be sent to you via e-mail to be signed prior to your visit. You will also be asked to send us any medical records for review.
Step 3:
You will have your virtual appointment. You can attend via video phone or your computer. All you need is access to a computer and privacy and any help if you need it. No downloads necessary!
